Keep form Inputs after submitting form (especially if errors are found)

I’m working on this website form: https://vecoklahoma.com/resources/#newpatientform

A) I need to figure out why it isn’t working properly. All other forms are submitting fine. This one isn’t. I’m sure I did something wrong.

B) If the form is submitted and has errors, how do I keep the user inputs so the user doesn’t have to put everything back in again?
